Customer Continuing (CE) Evaluation Forms Completion

1. Complete the following 2 sets of forms:

• CE_Your Last_First_Last 4 of your SSN_SIG
• CE_Your Last_First_last 4 of your SSN_FBI

2. Rename each form as described

• CE_Your Last_First_Last 4 of your SSN_SIG
  Example: CE_Jones_Bob_9090_SIG
• CE_Your Last_First_last 4 of your SSN_FBI
  Example: CE_Jones_Bob_9090_FBI

3. Return the completed forms as two separate files to ESSS@ngc.com, and include your MYID and the words "Customer CE" in the subject line.
